Calendar Year Deductible - Medical
(Active and Retiree Plans (without Medicare))

Non-contract physicians - There is a $250 per person deductible with a maximum of $750 per family.
 Contract physicians** - There is no deductible for contract physician claims.

 **Contract physicians are:
 Residents of California - Anthem Blue Cross Prudent Buyer providers.
 Residents of Nevada - Anthem Blue Cross/Blue Shield providers.
 Residents elsewhere - AHF providers, where available.

PROVIDERS: If you are a provider, your Federal Tax ID number and the participant's

 Social Security number or HCID number are required. You will not be able to access 
 the files if the Federal Tax  ID number and identification numbers are not valid or do not match
 our records.
