Operating Engineers Health & Welfare Fund


NOTICE

As you know, the Health & Welfare Fund has been very slow in processing provider claims due to cash flow shortfalls arising from the decline in work hours. The Board of Trustees has approved a plan to reduce the claims backlog. The Fund Office will take between 60 and 90 days to process the current backlog. Please be patient as we work to resolve this matter.

 

 

CHECK TO SEE IF A CLAIM WAS PAID

Calendar Year Deductible - Medical
(Active and Retiree Plans (without Medicare))

In-Network (Contract) facility/physicians* - There is a $350 per individual Calendar Year Deductible with a maximum of $1,050 per family.  (Applicable to services dates of 01/01/2011 and after)

 

Out-of-Network (Non-Contract) facility/physicians - There is a $500 per individual Calendar Year Deductible with a maximum of $1,500 per family.  (Applicable to services dates of 01/01/2011 and after)

 

*In-Network (Contract) facility/physicians are provided as follows:
 Residents of California - Anthem Blue Cross Prudent Buyer providers.
 Residents of Nevada - Anthem Blue Cross/Blue Shield providers.
 Residents elsewhere - AHF providers, where available.

PROVIDERS: If you are a provider, your Federal Tax ID number and the participant's

 Social Security number or HCID number are required. You will not be able to access 
 the files if the Federal Tax  ID number and identification numbers are not valid or do not match
 our records.
